Pre-admission (for freshmen, transferees, second degree applicants and foreign students; not applicable to College of Law, Graduate School and CLEP applicants)

1. Presents requirements to the Admission and Information Staff/Assistant/CPAD.

2. Refers foreign applicants to SRMD for evaluation of credentials (for foreign students only).
3. Refers applicant for interview by the Dean (for transferees, second degree applicants who are graduates of two (2) or three (3) year courses, and BS Nursing applicants.)

4. Refers applicant to the SRMD Evaluator/Staff for Evaluation of credited subjects (for transferees and second degree applicants).

5. Evaluates units to be credited (for transferees and second degree applicants).

7. Processes applicant’s CET Application.
8. Pays corresponding CET fee at the Cashier’s Office.

12. Issues Letter of Acceptance to applicant for submission to Bureau of Immigration (BI). (For foreign students only).

13. Processes Student Visa at the BI (For foreign students only)
14. (For BS Nursing and CITHM applicants only) Proceeds to the Health and Services Department (HSD) for medical/dental check-up.

Admission Procedures for Freshmen (undergraduate)

Responsibilities / Authorities

1. Submits the College Entrance Test (CET) result and other required credentials to the Admission staff of the Student Records Management Department (SRMD) – Office of the University Registrar.
2. Accomplishes the Student Application for Admission (F-REG-001) which contains his/her personal information.
3. Signs an undertaking not to join unrecognized organizations and fraternities/sororities in LPU (F-REG-002).

4. On-line enlistment at the CPAD
5. Gives the student a copy of the Enrolment Procedure for Freshmen which also contains the dates of freshmen orientation and opening of classes.

6. Proceeds to the Accounting Department for payment of fees (partial or full payment) and issuance of Enrollment Assessment Form (EAF).
7. Proceeds to the Purchasing Office/Bookstore to order school/P.E. uniform.
8. Proceeds to the Information and Communication Technology Department (ICTD) for School I.D. picture taking.
9. Proceeds to the HSD for medical/dental check-up except for BS Nursing and CITHM enrollees.

 

Admission Procedures for Transferees and Second Degree Applicants (undergraduate)

1. Submits the CET result and other required credentials to the Admission staff of the Student Records Management Department (SRMD) – Office of the University Registrar.
2. Accomplishes the Student Application for Admission (F-REG-001) which contains his/her personal information.

4. Issues to the student his permit to enroll and Enrolment Procedure for Second Degree Applicants/Transferees which also contains the dates of orientation and opening of classes.

5. Proceeds to the college and presents the accomplished Plotting Form, permit to enroll, and evaluation results of credited subjects to enrolment adviser.
6. Proceeds to the Accounting Department for payment of fees (partial or full payment) and issuance of Enrollment Assessment Form (EAF).
7. Proceeds to the Purchasing Office/Bookstore to order school/P.E. uniform.
8. Presents the OR/EAF to the Information and Communication Technology Department for School I.D picture taking.
9. Proceeds to the School Clinic for Medical/Dental check-up, except for BS Nursing and CITHM enrollees.

 

Admission Procedures for the Graduate School

1. Submits Transcript of Records/True Copy of Grades and other required credentials to the Graduate School Evaluator at the SRMD.

Evaluates subjects taken from previous school (for transferees or second degree applicants only)

2. Proceeds to the Cashier’s Office to pay the entance exam fee.
3. Reports to Guidance and Testing Center for the written Admission Test.

4. (For foreigners only) Pays the corresponding Acceptance Fee except for Ph.D. and graduate school students under customized programs.
5. If complied with all admission the requirements, accomplishes the Student Application for Admission (F-REG-001) which contains his/her personal information at the SRMD – Office of the University Registrar.

6. Issues to the students his/her student number/permit to enroll and Enrolment Procedure. Enlists the student.

Admission Procedures for the College of Law

1. Submits Transcript of Records bearing CHED Special Order Number or exemption from issuance of such reflecting graduation from baccalaureate degree and other required documents to the COL staff.
2. Takes the entrance exam.
3. Accomplishes the Student Application for Admission-College of Law (F-REG-048) which contains his/her personal information.
